Welcome to St. David’s Consultation Services
Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS will replace personal care assistance (PCA) and the Consumer Support Grant (CSG).
At St. David’s, we empower individuals to fully embrace and experience their unique strengths, goals, and choices. Our person-centered CFSS consultation services are designed to help you, or your loved one, navigate options and identify the tools, resources, and services that best align with your needs.
As a first step, we’ll need a service agreement that names St. David’s as your chosen CFSS consultation provider. You can let your assessor or case manager know, and they will generate the agreement you need to get started.

Along with eligibility criteria below, the CFSS program cannot serve clients already receiving St. David's Case Management.
Eligibility
Client Must:
- Use one of the following Minnesota Health Care Programs:
- Alternative Care (AC).
- Home and community-based services (HCBS) waivers.
- Medical Assistance (MA).
- Pregnant people and people under the age of 19 who are on MinnesotaCare.
- Not live in any of the following settings:
- Hospital.
- Nursing facility.
- Intermediate care facility for persons with developmental disabilities (ICF/DD).
- Foster care setting licensed for more than six people.
- Not live in housing owned or controlled by their PCA/CFSS provider agency
- Be able to direct their own care (or have an RP/representative do so on their behalf), which means they can:
- Identify their own needs.
- Direct and evaluate worker tasks and accomplishments.
- Provide and arrange for their own health and safety.
- Receive an assessment, as described on CFSS Manual – Assessment for PCA/CFSS services, that determines at least one of the following is true:
- The person requires hands-on assistance or cueing and supervision to complete at least one activity of daily living (ADL) (refer to CFSS Manual – PCA/CFSS covered services).
- The person has a level I behavior (i.e., physical aggression toward themselves or others or destruction of property that requires an immediate response from another person).
- Not use the Family Support Grant

You have the right to make informed choices about your care and to receive services that reflect your goals and values. Your providers are responsible for delivering services with respect, transparency, and collaboration.
To receive PCA/CFSS services, individuals must meet specific criteria outlined by the Minnesota Department of Human Services.
Agency Model
In this model, the CFSS provider agency is the employer of your support workers. The agency handles hiring, training, supervision, and payroll. You and the agency work together to monitor the effectiveness of your service plan. If you plan to purchase goods or services, you’ll also need to select a Financial Management Services (FMS) provider.
Budget Model
Here, you (or your representative) are the employer. You recruit, hire, train, and supervise your support workers. You’ll also choose an FMS provider to assist with employer-related tasks. You are responsible for monitoring your service plan’s effectiveness.
No. Consultation Services are not part of your CFSS budget and do not reduce your assessed allocation. During your first 12 months, you’ll receive two service authorization periods:
- A 90-day authorization with 6 sessions
- A 9-month authorization with 6 additional sessions
Our Consultation Services include:
- Service set-up and annual CFSS orientation
- Ongoing consultation and skill-building
- Quality assurance and support throughout your journey
